Welcome to the CAVS Annual Membership online signup form.

The 2009-2010 annual membership cost is $ 225.00 for the year.  The fee includes the players practice uniform and two CAVS t-shirts. Please be sure to fill in all of the requested information below.   It is extremely important that you select the correct size for your uniform and t-shirts sizes.
